DIABETES First Line Therapies Insulin Therapy > Insulin Glargine is a long-acting basal insulin that provides stable, continuous blood sugar control with a single daily injection. This insulin formulation is effective for both Type 1 and Type 2 diabetes patients. It plays a crucial role in managing blood sugar levels throughout the day and especially overnight when used in combination with prandial insulin for meal-time control.
Insulin Glargine offers long-lasting glucose control by mimicking the natural insulin released by the body. It is unique in that it provides a steady insulin level over 24 hours, with no pronounced peaks or troughs. This insulin formulation helps prevent blood sugar spikes and dips, particularly during the night, and is essential for patients requiring stable basal insulin levels as part of their diabetes management.
For Type 1 Diabetes - Insulin Glargine (U-100): - Initial Dose: Approximately 1/3 of total daily insulin requirement, administered once daily. - Maintenance Dose: Adjust according to metabolic needs and blood glucose levels. - Insulin Glargine (U-300): - Initial Dose: 1/3 to 1/2 of total daily insulin requirement, administered once daily. Full effect may take 5 days. - Maintenance Dose: Adjust based on individual needs, with dose titration every 3-4 days. - Dose Conversion (U-100 to U-300): U-300 requires a higher dose to maintain similar glycemic control. - Conversion from other insulins: Adjust dose based on the type of insulin being switched. For Type 2 Diabetes - Insulin-naive: - U-100: Start at 0.2 units/kg (up to 10 units) daily. - U-300: Start at 0.2 units/kg daily. The effect may take 5 days to manifest. - Dose Conversion: Switch between U-100 and U-300 as per the guidelines for Type 1 diabetes. For Pediatric Type 1 and Type 2 Diabetes (Age 6 and older) - Insulin Glargine (U-100 or U-300): - Initial Dose: One-third of the total daily insulin requirement. - Maintenance Dose: Adjust according to needs and blood glucose levels. - Conversion: Similar to adult dosing, with extra caution in pediatric cases. General Comments: - Changes in physical activity, diet, or illness may require dose adjustments. - Blood glucose should be closely monitored, particularly during insulin regimen changes. - For basal-bolus therapy, insulin glargine provides one-third to one-half of the total insulin requirement, with bolus insulin for the remaining portion. ------------------ How to Use the Lantus SoloStar Pen in 6 Steps 1. Get Ready: - Remove the pen cap. - Check the insulin in the reservoir for clarity (should be clear and colorless). - Never share needles or reuse them. 2. Attach the Needle: - Wipe the pen tip with an alcohol swab. - Attach the new needle, ensuring it is straight and secure. - Remove the outer needle cap and save it for later. - Discard the inner needle cap. 3. Perform a Safety Test: - Dial a test dose of 2 units. - Hold the pen with the needle pointing up and tap the insulin reservoir to release air bubbles. - Press the injection button and check if insulin is released. Repeat if necessary. 4. Select the Dose: - Ensure the dial is set to “0” before selecting your dose. - Adjust if necessary and check for enough insulin in the pen. 5. Inject Your Dose: - Clean the injection site with an alcohol swab. - Keep the pen straight, insert the needle, and press the injection button. - Count to 10 to ensure the full dose is administered before removing the needle. 6. Remove the Needle: - After injecting, remove the needle and dispose of it in a sharps container. - Store the pen at room temperature with the cap back on.
Store the insulin pen in the refrigerator before use. After opening, it can be kept at room temperature for up to 28 days. Do not freeze the pen, and protect it from direct sunlight and heat.
Common side effects include injection site reactions such as redness, swelling, or pain. Other potential side effects include hypoglycemia (low blood sugar), weight gain, and fluid retention. Rarely, allergic reactions can occur. Prolonged use may cause lipodystrophy (fatty tissue changes at the injection site).
