Usual Adult Dose for Alcohol Dependence (Intramuscular): → 380 mg intramuscularly once every 4 weeks Usual Adult Dose for Opiate Dependence (Oral): → Initial dose: 25 mg orally once daily → Maintenance dose: 50 mg orally once daily (if no withdrawal occurs at initial dose) Usual Adult Dose for Opiate Dependence (Intramuscular): → 380 mg intramuscularly once every 4 weeks Comments: - Patients must be opioid-free for at least 7–14 days before initiating naltrexone to avoid precipitated withdrawal. - Transitioning from methadone or buprenorphine should be done cautiously with symptomatic management using non-opioid medications. - Success of therapy is heavily dependent on adherence and integration into a broader psychosocial treatment plan.
