Naltrexone is a pure opioid receptor antagonist with a high affinity for the mu-opioid receptor. Usual Adult Dose for Alcohol Dependence (Oral): → 50 mg orally once daily Duration: Typically 12 weeks, though extended treatment may benefit long-term abstinence. Usual Adult Dose for Alcohol Dependence (Intramuscular): → 380 mg IM every 4 weeks / once monthly Usual Adult Dose for Opiate Dependence (Oral): → Initial dose: 25 mg orally once daily → Maintenance dose: 50 mg orally once daily (if no signs of withdrawal at 25 mg dose) Usual Adult Dose for Opiate Dependence (Intramuscular): → 380 mg IM every 4 weeks / once monthly Comments: - Patients transitioning from opioid agonists (e.g., methadone, buprenorphine) to naltrexone must allow a minimum of 7–14 days opioid-free to avoid precipitated withdrawal. - Non-opioid medications may be required to manage withdrawal symptoms during the transition. - Naltrexone is most effective when used as part of a compliance-focused, multidisciplinary recovery plan involving therapy and close supervision.
