Paroxetine acts by selectively inhibiting serotonin reuptake transporters in the brain, increasing serotonin levels at neuronal synapses, which results in improved communication and relief from symptoms of depression, anxiety, and related disorders.
Adult Dosing - Depression & Major Depressive Disorder (MDD): Immediate Release (IR): Initial dose 20 mg daily; maintenance 20–50 mg, max 50 mg. Controlled Release (CR): Initial dose 25 mg daily; maintenance 25–62.5 mg, max 62.5 mg. Dose increments by 10 mg (IR) or 12.5 mg (CR) weekly are common. Treatment duration usually spans several months, with proven efficacy for up to one year. - Social Anxiety Disorder (SAD): IR: Start 20 mg daily; maintain 20–60 mg, max 60 mg. CR: Start 12.5 mg daily; maintain 12.5–37.5 mg, max 37.5 mg. Typically treated for up to 12 weeks with weekly dose adjustments. - Panic Disorder (PD): IR: Start 10 mg daily; maintain 10–40 mg, max 60 mg. CR: Start 12.5 mg daily; maintain 12.5–75 mg, max 75 mg. Treatment usually lasts up to 12 weeks, with weekly dose increases. - Premenstrual Dysphoric Disorder (PMDD): CR only, dosed 12.5–25 mg daily on continuous or cyclic schedules.
