Arkacan 100 is an oral medication containing clonidine 150mg, a centrally acting alpha-2 adrenergic agonist primarily used for its antihypertensive and sedative effects. Clonidine reduces sympathetic outflow from the central nervous system, helping to lower blood pressure and calm hyperactive neural circuits. It has been repurposed for several psychiatric uses, including ADHD, due to its ability to regulate impulsivity and improve sleep quality.

Clonidine stimulates alpha-2 adrenergic receptors in the brainstem, reducing norepinephrine release and lowering peripheral vascular resistance. This leads to decreased blood pressure and calming of hyperactive neuronal pathways. Its sedative and analgesic properties make it useful in managing ADHD, hypertension, and certain types of chronic pain.
Hypertension – Oral: - Start with 0.1 mg twice daily (morning and bedtime) - Titrate weekly by 0.1 mg/day increments as needed - Maintenance: 0.2 to 0.6 mg/day in divided doses - Max: 2.4 mg/day Hypertension – Transdermal Patch: - Start with 0.1 mg/day patch applied every 7 days - Increase if needed by adding or using higher strength patches - Max: Two 0.3 mg patches per 7 days Pain (Epidural Infusion for Cancer Pain): - Initial: 30 mcg/hr - Max: 40 mcg/hr - Used in conjunction with opioids for neuropathic pain ADHD (Children 6+ years, Extended Release): - Initial: 0.1 mg at bedtime - Titrate by 0.1 mg/day every 7 days - Max: 0.4 mg/day in 2 doses (split equally or higher dose at night) - Tablets must be swallowed whole. Do not chew/crush. - When stopping, taper by 0.1 mg every 3–7 days to avoid rebound hypertension.
Store in a cool, dry place away from light and moisture. Keep tightly sealed and out of reach of children. Avoid high humidity environments.
- Common: Dry mouth, drowsiness, dizziness, constipation - Less Common: Bradycardia, fatigue, irritability, rebound hypertension (if discontinued abruptly) - Rare but Serious: Depression, cardiac conduction issues, withdrawal symptoms upon abrupt cessation
