Hypertension – Oral: - Start with 0.1 mg twice daily (morning and bedtime) - Titrate weekly by 0.1 mg/day increments as needed - Maintenance: 0.2 to 0.6 mg/day in divided doses - Max: 2.4 mg/day Hypertension – Transdermal Patch: - Start with 0.1 mg/day patch applied every 7 days - Increase if needed by adding or using higher strength patches - Max: Two 0.3 mg patches per 7 days Pain (Epidural Infusion for Cancer Pain): - Initial: 30 mcg/hr - Max: 40 mcg/hr - Used in conjunction with opioids for neuropathic pain ADHD (Children 6+ years, Extended Release): - Initial: 0.1 mg at bedtime - Titrate by 0.1 mg/day every 7 days - Max: 0.4 mg/day in 2 doses (split equally or higher dose at night) - Tablets must be swallowed whole. Do not chew/crush. - When stopping, taper by 0.1 mg every 3–7 days to avoid rebound hypertension.
