Adult Dosing: - Epilepsy: Titration required; usual maintenance 100–500 mg/day based on co-administered drugs. - Bipolar Disorder: Start 25 mg/day; target 200 mg/day (adjust for valproate or inducers). - Lennox-Gastaut: Similar titration to epilepsy depending on combination therapy. Pediatric Dosing: - 2–12 years: - With valproate: 1–5 mg/kg/day (max 200 mg/day). - Without modifiers: 4.5–7.5 mg/kg/day (max 300 mg/day). - With inducers: 5–15 mg/kg/day (max 400 mg/day). - Over 12 years: Dose similar to adults, adjusted by body weight.
