  • Epilepsy – Adjunctive (Extended-Release): Start: 25–50 mg once daily, titrate to 200–400 mg/day.
  • Migraine Prophylaxis – Immediate-Release: Week 1: 25 mg at bedtime; increase weekly to 50 mg twice daily (100 mg/day) by Week 4.
  • Migraine Prophylaxis – Extended-Release: Week 1: 25 mg/day; increase weekly to 100 mg/day by Week 4.
  • Lennox-Gastaut Syndrome – Immediate/Extended-Release: Start: 25–50 mg/day; titrate weekly to 200–400 mg/day in 2 divided doses.
