Epilepsy (Adjunctive) - IR: 200–400 mg/day in divided doses • Start: 25–50 mg/day → Increase by 25–50 mg/week - ER: Same target once daily 3. Migraine Prophylaxis - IR: 100 mg/day (50 mg BID) • Week 1: 25 mg at bedtime → Titrate to 50 mg BID by Week 4 - ER: 100 mg/day once daily 4. Lennox-Gastaut Syndrome - IR/ER: 200–400 mg/day • Start at 25–50 mg/day → Increase weekly [Smoking Cessation (Off-label Use)] - Week 1: 25 mg once daily at bedtime - Week 2: 50 mg once daily - Week 3: 75 mg once daily - Week 4: 100 mg once daily - Week 5: 150 mg once daily - Week 6: 200 mg once daily - Week 7–8: Up to 300 mg once daily if tolerated Duration: Continue for 8–12 weeks based on response.
