It contributes to mood stabilization and emotional regulation in patients with chronic anxiety or PTSD. > ADHD (Attention Deficit Hyperactivity Disorder) In children and adults with coexisting epilepsy or mood disorders, lamotrigine may be used off-label to support attention regulation and emotional balance. > Improve Memory, Concentration and Brain Function Clinical observations suggest benefits in enhancing cognitive performance, including attention, concentration, and working memory, making it a supportive option for neurological conditions beyond epilepsy.
Lamotrigine acts by inhibiting voltage-sensitive sodium channels and reducing excessive release of excitatory neurotransmitters such as glutamate. This helps stabilize neuronal membranes, suppress abnormal discharges, and regulate emotional states in mood disorders.
Adult Dose - Epilepsy (Adjunctive Therapy) • With valproate: 25 mg every other day (Week 1–2) → 25 mg daily (Week 3–4) → Maintain 100–200 mg/day • No inducers/inhibitors: 25 mg daily (Week 1–2) → 50 mg daily (Week 3–4) → Maintain 225–375 mg/day • With enzyme inducers: 50 mg daily (Week 1–2) → 100 mg daily (Week 3–4) → Maintain 300–500 mg/day - Bipolar Disorder • With valproate: Start 25 mg every other day → Titrate to 50 mg → Maintain at 100 mg/day • No inducers/inhibitors: Start at 25 mg daily → Titrate to 100 mg → Maintain at 200 mg/day • Enzyme inducers: Begin at 50 mg → Escalate to 300 mg → Maintain up to 400 mg/day - Lennox-Gastaut Syndrome • Titration mirrors epilepsy guidelines • Final dose depends on interacting drugs: - With valproate: 100–200 mg/day - No inducers/inhibitors: 225–375 mg/day - With inducers: 300–500 mg/day Pediatric Dose - Age 2–12 years • With valproate: Start at 0.15 mg/kg/day → Increase to 0.3 mg/kg/day → Titrate to 1–5 mg/kg/day (Max: 200 mg) • No inducers/inhibitors: Start at 0.3 mg/kg/day → Titrate to 4.5–7.5 mg/kg/day (Max: 300 mg) • Enzyme inducers: Start at 0.6 mg/kg/day → Titrate to 5–15 mg/kg/day (Max: 400 mg) - Age 13+ years • Follow adult XR titration schedule - Under 2 years • Not recommended - Monotherapy Conversion • From older AEDs: Slowly discontinue over 4 weeks while titrating lamotrigine to 500 mg/day • From valproate: Titrate lamotrigine while tapering valproate, achieving 500 mg/day lamotrigine monotherapy - IR to XR Conversion • Allowed as a direct switch • Monitor closely for seizure control • Adjust dose as needed, especially with enzyme inducers
