Adult Dose: - Epilepsy (Adjunctive): • With valproate: 25 mg every other day (Week 1–2) → 25 mg daily (Week 3–4) → Maintain 100–200 mg/day • No enzyme inducers/inhibitors: 25 mg daily → 50 mg daily → Maintain 225–375 mg/day • With enzyme inducers: 50 mg daily → 100 mg daily → Maintain 300–500 mg/day - Bipolar Disorder: • With valproate: 25 mg every other day → 25 mg daily → 50 mg → Maintain 100 mg/day • No enzyme modifiers: 25 mg daily → 50 mg → 100 mg → Maintain 200 mg/day • With inducers: 50 mg → 100 mg → 200 mg → 300 mg → Maintain up to 400 mg/day - Lennox-Gastaut Syndrome: • Follow epilepsy dosing based on adjunctive protocol Pediatric Dose: - Age 2–12: • With valproate: Start at 0.15 mg/kg/day → Increase to 1–5 mg/kg/day (Max 200 mg/day) • No modifiers: Start at 0.3 mg/kg/day → 4.5–7.5 mg/kg/day (Max 300 mg/day) • With inducers: Start at 0.6 mg/kg/day → 5–15 mg/kg/day (Max 400 mg/day) - Age 13+: • Follow adult XR dosing per concomitant drug profile - Under 2: • Not recommended Monotherapy Conversion (≥16 years): - From carbamazepine/phenytoin/etc: Titrate lamotrigine to 500 mg/day, taper existing AED by 20% weekly - From valproate: Increase lamotrigine gradually to 200–300–500 mg/day, taper valproate in stages IR to XR Conversion: - Patients may switch directly. Monitor for seizure control. Enzyme inducers may require dose adjustments.
Store Lamez 25 in a cool, dry place at 15–30°C, away from sunlight and moisture. Keep tablets in their original packaging. Do not freeze or expose to humidity. Ensure child-resistant storage.
Common side effects include dizziness, headache, blurred vision, nausea, and fatigue. Severe reactions may involve skin rash, including life-threatening conditions like Stevens-Johnson syndrome. Risk of rash is minimized by slow dose escalation. Patients should report any allergic symptoms or skin changes immediately.
