Adult Dose – Epilepsy (Adjunctive Therapy) • With valproate: 25 mg every other day (Week 1–2) → 25 mg daily (Week 3–4) → Maintenance: 100–200 mg/day • Without inducers/inhibitors: 25 mg daily (Week 1–2) → 50 mg daily (Week 3–4) → Maintenance: 225–375 mg/day • With enzyme inducers: 50 mg daily (Week 1–2) → 100 mg daily (Week 3–4) → Maintenance: 300–500 mg/day Adult Dose – Bipolar Disorder (Maintenance Therapy) • With valproate: 25 mg every other day → 25 mg daily → 50 mg → Maintenance: 100 mg/day • Without inducers/inhibitors: 25 mg → 50 mg → 100 mg → Maintenance: 200 mg/day • With enzyme inducers: 50 mg → 100 mg → 200 mg → 300 mg → Maintenance: up to 400 mg/day Note: XR not approved for bipolar disorder. Adult Dose – Lennox-Gastaut Syndrome • Same titration as for epilepsy • Maintenance based on concurrent medication: - With valproate: 100–200 mg/day - Without inducers/inhibitors: 225–375 mg/day - With inducers: 300–500 mg/day Pediatric Dose – Epilepsy (2–12 years) • With valproate: Start 0.15 mg/kg/day → Increase 0.3 mg/kg/week → Maintenance 1–5 mg/kg/day (Max 200 mg) • Without inducers/inhibitors: Start 0.3 mg/kg/day → Increase 0.6 mg/kg/week → Maintenance 4.5–7.5 mg/kg/day (Max 300 mg) • With inducers: Start 0.6 mg/kg/day → Increase 1.2 mg/kg/week → Maintenance 5–15 mg/kg/day (Max 400 mg) Pediatric Dose – Age 13+: Follow adult XR regimen based on other medications used Pediatric Dose – Lennox-Gastaut Syndrome: Same as pediatric epilepsy protocol Not recommended under 2 years Monotherapy Conversion (Adults ≥16 years): • From carbamazepine, phenytoin, phenobarbital, or primidone: Titrate to 500 mg/day while tapering old AED • From valproate: Titrate to 200 mg/day, taper valproate, increase lamotrigine to 500 mg/day IR to XR Conversion: Direct switch possible Monitor seizure control; adjust dose for enzyme inducers
