Donamem 5 is a fixed-dose combination tablet containing donepezil 5mg and memantine 5mg, developed for the treatment of moderate to severe Alzheimer’s disease. This dual-action therapy targets two distinct yet complementary pathways involved in neurodegeneration: cholinergic dysfunction and glutamatergic excitotoxicity. Donepezil works by inhibiting acetylcholinesterase, thereby preserving acetylcholine levels essential for learning and memory, while memantine selectively blocks NMDA receptors to reduce overstimulation by glutamate, protecting neurons from further damage. Donamem 5 is particularly beneficial in patients whose symptoms are no longer adequately managed by monotherapy. It offers enhanced symptom control by improving memory, orientation, verbal ability, and reducing behavioral disturbances such as aggression, disorientation, and apathy. The synergistic effect of these two agents not only slows cognitive decline but also helps patients maintain independence in their daily activities for a longer period.

Donamem 5 utilizes the complementary pharmacological actions of donepezil and memantine. Donepezil increases acetylcholine availability in the synaptic cleft by inhibiting its enzymatic breakdown, thereby improving communication between neurons responsible for memory and cognition. Memantine, on the other hand, blocks overactive NMDA receptors triggered by excessive glutamate, reducing neuronal damage caused by calcium overload. This dual mechanism is especially useful in moderate to advanced stages of Alzheimer’s disease, where multiple neurotransmitter systems are compromised. Donamem 5 is considered safe for long-term use and does not cause physical dependence.
For patients stabilized on memantine (10 mg twice daily or 28 mg ER once daily) and donepezil 10 mg once daily: → Switch to memantine 28 mg ER–donepezil 10 mg fixed-dose combination taken once daily in the evening Patients with severe renal impairment: → Use memantine 5 mg twice daily or 14 mg ER once daily + donepezil 10 mg once daily before transitioning Begin this combination therapy the day after the last separate doses of memantine and donepezil have been administered. Take exactly as prescribed. Do not crush or split the tablet. Can be taken with or without food.
