The recommended starting dose of memantine hydrochloride is 5 mg once daily. → Titrate upward in 5 mg increments at intervals of at least 1 week: - 10 mg/day (5 mg twice daily) - 15 mg/day (5 mg and 10 mg separately) - 20 mg/day (10 mg twice daily) — optimal dose shown effective in clinical trials Memantine may be taken with or without food.
