Product Description

Monurol Granule 3g Sachet contains fosfomycin, a broad-spectrum bactericidal antibiotic that targets bacterial cell wall synthesis by inactivating enolpyruvyl transferase (MurA). This prevents peptidoglycan formation, leading to bacterial cell death.

Its single-dose oral formulation provides high urinary concentrations for 48–72 hours, making it highly effective for uncomplicated urinary tract infections (UTIs).

Fosfomycin is also valuable against resistant pathogens, including ESBL-producing E. coli and carbapenem-resistant Enterobacteriaceae (CRE), and is used intravenously in hospital-acquired infections like sepsis or pneumonia caused by multidrug-resistant organisms.

Indications & Uses

  • Antibiotics & Infections | Urinary Tract Infection (UTI): First-line therapy for acute uncomplicated cystitis in women. Single-dose oral administration ensures bactericidal urinary levels with minimal systemic exposure.
  • Systemic Infection: Intravenous fosfomycin is reserved for complex infections involving multidrug-resistant pathogens, often in combination therapy for bacteremia, ventilator-associated pneumonia, and intra-abdominal abscesses.
  • MDR Organism Coverage: Effective against ESBL-producing E. coli, CRE, and other resistant uropathogens, preserving other antibiotic options and reducing carbapenem reliance.
  • Combination Therapy: Enhances efficacy against MRSA, Pseudomonas, and CRE when combined with beta-lactams or aminoglycosides, reducing resistance development.
  • Outpatient Convenience: Ideal for outpatient cystitis due to single-dose oral format, improving adherence and minimizing pill burden.

Benefits & Effectiveness

  • Mechanism: Inhibits MurA enzyme, blocking the first step of peptidoglycan synthesis → rapid bactericidal action.
  • Spectrum: Broad-spectrum activity against Gram-negative and Gram-positive bacteria, including resistant strains.
  • Resistance profile: Unique mechanism prevents cross-resistance, supporting monotherapy or synergistic combination protocols for resistant infections.

Directions for Use

  • Adults – Acute Uncomplicated Cystitis: 3 g orally once (single dose).
  • Administration: Dissolve contents of one sachet in a glass of water and take immediately.
  • Timing: Take on an empty stomach, preferably at bedtime after urination.
  • Limitations: Not recommended for pyelonephritis or perinephric abscess.
  • Indication: Only for lower urinary tract infections caused by susceptible E. coli or Enterococcus faecalis.
  • Repeat dosing: Do not repeat dose unless directed by a physician.
  • Follow-up: If symptoms persist >72 hours, seek further evaluation.
  • Drug interactions: Avoid co-administering drugs that reduce urinary concentration or increase renal excretion.
  • Renal/Hepatic Impairment: Use caution in significant renal dysfunction; dose adjustment generally not needed for single-dose therapy.

Storage

  • Storage condition: Store below 25 °C in a dry place, away from moisture and direct sunlight.
  • Packaging: Keep sachets sealed until use.
  • Safety: Keep out of reach of children.

Possible Side Effects

  • Common: headache, diarrhea, nausea, abdominal pain, dizziness.
  • Less common: vulvovaginitis, fatigue, back pain, flatulence.
  • Rare but serious: allergic reactions including rash, angioedema, or anaphylaxis.
  • Action: Discontinue immediately and consult a doctor if rash, swelling, or difficulty breathing occurs.
