Niftran 100 Capsule contains nitrofurantoin, a urinary tract–specific antibiotic that works by damaging bacterial DNA through reactive intermediates generated inside the cell. It is particularly effective for the treatment and prevention of uncomplicated lower urinary tract infections (UTIs), especially acute cystitis caused by common pathogens such as Escherichia coli and Enterococcus faecalis. Nitrofurantoin achieves high concentrations in the urine while maintaining low systemic levels, making it ideal for localized bladder infections with minimal impact on the gut microbiota. Due to its targeted action, it is often used in long-term prophylaxis to prevent recurrent UTIs in patients with normal renal function. It is contraindicated in patients with significant renal impairment, late-term pregnancy, and in certain cases of systemic infection where tissue penetration beyond the urinary tract is required.

Particularly beneficial for individuals with frequent post-coital or idiopathic urinary infections. > Targeted Therapy in Resistant Strains Retains efficacy against many multidrug-resistant E. coli strains, offering an alternative where other oral agents have failed. > Limited Use in Upper Tract Infections Not indicated for pyelonephritis or perinephric abscess due to insufficient tissue levels in the kidneys and surrounding structures.
Nitrofurantoin is reduced by bacterial flavoproteins to reactive intermediates that attack multiple bacterial macromolecules, particularly DNA, leading to cell death. It demonstrates bactericidal activity within the urinary tract and is highly selective for urinary pathogens. Rapid urinary excretion ensures high localized drug concentration, making it suitable for bladder-level infections but not systemic or upper urinary tract infections. It has a low propensity for resistance development when used appropriately.
Adults → Acute Uncomplicated Cystitis: 100 mg orally twice daily for 5 days (macrocrystal/monohydrate form) or 50–100 mg four times daily for 5–7 days (immediate-release form) → For single-dose regimens in select protocols, 3 g orally once (as per certain guidelines, not standard for nitrofurantoin) → Take with food to improve absorption and reduce gastrointestinal upset Prophylaxis of Recurrent UTIs → 50–100 mg orally at bedtime daily for up to 6–12 months under medical supervision Notes: → Not indicated for pyelonephritis or perinephric abscess → Use only in patients with creatinine clearance ≥60 mL/min for optimal efficacy → Effective primarily against E. coli, E. faecalis, and some Gram-negative rods → Avoid use in late pregnancy (38–42 weeks gestation) due to risk of neonatal hemolysis → Not recommended in infants under 1 month of age Renal/Hepatic Impairment: → Contraindicated in severe renal impairment (CrCl <60 mL/min) → Use cautiously in hepatic dysfunction
Store below 25°C in a cool, dry place away from moisture and heat. Keep capsules in their original packaging until ready to use. Keep out of reach of children.
Common: nausea, headache, flatulence, mild diarrhea Less common: dizziness, drowsiness, rash, abdominal discomfort Rare but serious: pulmonary hypersensitivity reactions, hepatotoxicity, peripheral neuropathy, hemolytic anemia (especially in G6PD deficiency) Seek immediate medical attention if experiencing breathing difficulty, persistent cough, jaundice, or numbness/tingling in extremities.
