Arpizol 10 is a generic form of aripiprazole, an atypical antipsychotic medication. It works by modulating dopamine and serotonin receptors in the brain to balance neurotransmitter activity. This helps in managing psychotic symptoms such as hallucinations and delusions, stabilizing mood swings in bipolar disorder, and augmenting antidepressant treatment in major depressive disorder patients with inadequate response.

Aripiprazole acts as a partial agonist at dopamine D2 and serotonin 5-HT1A receptors and antagonist at 5-HT2A receptors. This unique receptor activity helps restore neurotransmitter balance, reducing psychosis and mood instability while minimizing side effects common to typical antipsychotics.
Adult Dosing: - Schizophrenia: Oral immediate-release (IR) tablets starting at 10–15 mg daily; usual maintenance dose is 10–15 mg; maximum 30 mg daily. Extended-release intramuscular (IM) injections begin with 400 mg IM plus oral 10–20 mg daily for 14 days, followed by monthly maintenance doses of 300–400 mg IM. - Bipolar Disorder: Oral IR monotherapy starts at 15 mg daily; max 30 mg. Adjunct therapy with lithium or valproate starts at 10–15 mg daily; max 30 mg. - Agitated State: IM injection initial dose is 9.75 mg; maintenance dose ranges 5.25–15 mg; maximum 30 mg/day. Oral therapy should replace injections as soon as possible. - Depression (Adjunctive): Start at 2–5 mg daily, increase by up to 5 mg weekly; maintenance dose ranges 2–15 mg; maximum 15 mg. Pediatric Dosing: - Schizophrenia (ages 13–17): Start 2 mg daily, titrate to 5 mg after 2 days, then 10 mg after another 2 days; max 30 mg. - Bipolar Disorder (ages 10–17): Same titration schedule; target dose 10 mg; max 30 mg. - Autism (ages 6–17): Start 2 mg daily, increase to 5 mg, then 10 or 15 mg if needed; maintenance dose 5–15 mg; max 15 mg. - Tourette’s Syndrome (ages 6–18): - <50 kg: Start 2 mg, titrate to 5 mg; max 10 mg. - ≥50 kg: Start 2 mg, titrate to 5 mg, then 10 mg by day 8; max 20 mg. General Comments: - Oral tablets and solution are interchangeable mg-to-mg up to 25 mg (30 mg tablets correspond to 25 mg oral solution). - Dose increases are typically spaced 1–2 weeks to reach steady plasma levels. - Extended-release injections require initial oral supplementation and confirmation of tolerability before full switch. - Maintenance doses should be periodically reviewed for effectiveness and tolerability. - Avoid administering both extended-release formulations in the same muscle on the same day.

Common side effects include headache, nausea, insomnia, dizziness, and anxiety. Metabolic changes, weight gain, and extrapyramidal symptoms can occur but tend to be less severe than older antipsychotics. Serious but rare side effects include neuroleptic malignant syndrome and tardive dyskinesia, requiring immediate medical attention.
