Arpizol 5 is a low-dose generic formulation of aripiprazole, an atypical antipsychotic medication widely utilized in managing several psychiatric disorders. Aripiprazole acts as a partial agonist at dopamine D2 and serotonin 5-HT1A receptors and antagonist at 5-HT2A receptors. This unique mechanism restores balance in brain neurotransmitters, reducing psychotic symptoms and improving mood stability without causing significant sedation or weight gain typical of some older antipsychotics.
Adult Dosing: - Schizophrenia: Oral immediate-release tablets start at 10–15 mg daily; usual maintenance 10–15 mg; max 30 mg daily. Extended-release intramuscular (IM) injections start with 400 mg IM plus oral 10–20 mg daily for 14 days; monthly maintenance 300–400 mg IM. - Bipolar Disorder: Oral IR monotherapy starts at 15 mg daily; max 30 mg. Adjunct to lithium/valproate starts at 10–15 mg; max 30 mg. - Agitated State: IM injection initial dose 9.75 mg; maintenance 5.25–15 mg; max 30 mg/day. Oral therapy should replace injections ASAP. - Depression (Adjunctive): Start 2–5 mg daily, increase by ≤5 mg weekly; maintenance 2–15 mg; max 15 mg. Pediatric Dosing: - Schizophrenia (13–17 years): Start 2 mg daily, titrate to 5 mg after 2 days, then 10 mg after 2 more days; max 30 mg. - Bipolar Disorder (10–17 years): Same titration; target 10 mg; max 30 mg. - Autism (6–17 years): Start 2 mg daily, increase to 5 mg then 10 or 15 mg if needed; max 15 mg. - Tourette’s Syndrome (6–18 years): <50 kg start 2 mg, titrate to 5 mg; max 10 mg. ≥50 kg start 2 mg, titrate to 5 mg then 10 mg by day 8; max 20 mg. General Notes: - Oral tablets and solutions are interchangeable mg-to-mg up to 25 mg. - Dose increases typically spaced 1–2 weeks to allow steady-state levels. - Extended-release injections require initial oral supplementation and tolerability confirmation. - Maintenance doses should be periodically reassessed. - Avoid administering different extended-release formulations in the same muscle on the same day.

Common side effects include headache, nausea, vomiting, dizziness, insomnia, and anxiety.
