Azel 80 is a high-dose oral androgen receptor inhibitor containing enzalutamide, designed for the treatment of advanced prostate cancer. It disrupts the androgen signaling pathway at multiple levels, including preventing nuclear translocation and DNA binding of the AR. Azel 80 provides the same therapeutic benefits as the 40 mg formulation, allowing simplified dosing—two tablets once daily to achieve the standard 160 mg dose. It is a preferred agent in both metastatic and non-metastatic castration-resistant prostate cancer (CRPC), and also approved for castration-sensitive disease.
Inhibits androgen receptor (AR) signaling at multiple stages of the pathway, including ligand binding, nuclear translocation, and DNA interaction.
Blocks androgen receptor activation, effectively reducing tumor growth and slowing disease progression.
Demonstrated significant improvements in progression-free survival and overall survival in both metastatic and non-metastatic castration-resistant prostate cancer.

The usual adult dose for prostate cancer is 160 mg orally once daily, which is equivalent to taking two 80 mg tablets. The medication can be taken with or without food, but the tablets should always be swallowed whole without chewing or crushing. Therapy must be used in combination with a GnRH analog or following a bilateral orchiectomy. Treatment is continued until there is disease progression or the patient develops intolerable toxicity.
Long-term monitoring is essential, especially in elderly patients, due to the risk of fatigue, falls, and rare neurologic adverse events such as seizures or cognitive changes.
- Common: fatigue, hypertension, back pain, hot flashes, diarrhea - Less common: cognitive impairment, falls, fractures - Rare: seizures (especially in patients with risk factors) Monitor blood pressure and neurologic status regularly, especially with prolonged use.
