Samenza 40 is an oral antiandrogen therapy containing enzalutamide, a potent androgen receptor (AR) inhibitor. It is utilized across multiple stages of prostate cancer, offering a steroid-free treatment alternative that directly blocks AR signaling within tumor cells. Its mechanism disrupts the transcriptional activity of AR even in the presence of minimal androgen levels, thereby extending survival and delaying disease progression in both metastatic and non-metastatic prostate cancer.

Samenza 40 is also indicated in combination with ADT for high-risk metastatic hormone-sensitive prostate cancer. When added to standard hormonal therapy, it provides superior long-term tumor control and reduces disease-related mortality.
> Biochemical Recurrence (Off-label)
Although not formally approved, enzalutamide is under investigation for patients with biochemical recurrence (rising PSA without imaging evidence). This off-label use aims to delay clinical relapse and extend time to progression.
Samenza 40 blocks AR signaling regardless of circulating testosterone levels. It is effective as monotherapy without corticosteroids, improving PSA response, progression-free survival (PFS), and overall survival (OS). Its broad efficacy spans multiple prostate cancer phenotypes and stages, making it a versatile treatment option.
The usual adult dose for prostate cancer is 160 mg orally once daily, which is equivalent to taking four 40 mg capsules. The medication can be taken with or without food, but capsules must be swallowed whole without chewing, breaking, or dissolving. Treatment should be given concurrently with a GnRH analog, unless the patient has undergone a bilateral orchiectomy.
Therapy should be continued until disease progression or the development of unacceptable toxicity. Dose reduction may be necessary in the event of significant adverse effects, such as seizures or persistent fatigue.
Store below 25°C in a dry, cool location. Protect the medication from moisture and direct sunlight. Keep the bottle tightly closed when not in use. Ensure the product is kept out of reach of children, and dispose of expired capsules safely according to local regulations.
Common adverse effects include fatigue, hot flashes, musculoskeletal pain, and hypertension. Neurologic risks are rare but may involve seizures, particularly in patients with predisposing conditions.
