Flagyl 400 Tablet contains metronidazole, a nitroimidazole antimicrobial agent with both antibacterial and antiprotozoal properties. It works by inhibiting DNA synthesis in susceptible anaerobic organisms and protozoa, making it a versatile treatment option across multiple infectious diseases. Metronidazole is effective in treating a wide range of anaerobic bacterial infections, including intra-abdominal abscesses, pelvic infections, septicemia, and gynecological conditions such as bacterial vaginosis and trichomoniasis. It also plays a significant role in the management of protozoal diseases like giardiasis and amebiasis. Additionally, topical and gel formulations are used for inflammatory skin conditions such as rosacea and acneiform eruptions of parasitic origin.
- Antibiotics & Infections > Trichomoniasis Treats Trichomonas vaginalis infections, particularly in sexually active adults. Can be administered as a single 2g dose or as a 7-day oral regimen. Often used in partner treatment strategies to prevent reinfection. > Bacterial Vaginosis Used to restore the vaginal microbiota balance disrupted by anaerobic overgrowth. Available in both oral and intravaginal formulations. Reduces recurrence risk and improves discharge, odor, and irritation. > Abdominal Infection Effective for peritonitis, diverticulitis, appendicitis, and pelvic infections caused by anaerobic bacteria. Commonly used with ceftriaxone or other beta-lactam agents. > Systemic Infection Indicated for anaerobic bacteremia, pelvic inflammatory disease, and lung abscesses. Penetrates deep tissues and body fluids, including CNS and bones. > Parasitic Intestinal Infections Manages protozoal infections such as amebiasis and giardiasis. Reduces diarrhea, abdominal pain, and parasitic shedding. Suitable for travelers and immunocompromised patients. > Clostridium difficile Infection (CDI) Alternative to vancomycin in mild-to-moderate CDI cases when other options are unavailable. Suppresses toxin-producing Clostridioides difficile in the colon. > Surgical Prophylaxis Administered before colorectal surgeries to reduce anaerobic post-operative complications. Enhances surgical outcomes when combined with cephalosporins. > Rosacea & Parasitic Skin Conditions Topical metronidazole reduces erythema, papules, and inflammation in rosacea. Also used for parasitic skin conditions such as demodicosis.
Metronidazole disrupts nucleic acid synthesis by converting to active metabolites in anaerobic environments, selectively targeting DNA of susceptible organisms. This unique mechanism allows for focused eradication of pathogens with minimal impact on aerobic flora. It is highly bioavailable, penetrates well into tissues and fluids, and is effective both as monotherapy and in combination regimens. The drug is not associated with physical dependence and can be used safely over extended periods in chronic infections or maintenance phases.
Anaerobic Bacterial Infections → Initial: 15 mg/kg IV over 1 hour → Maintenance: 7.5 mg/kg IV or orally every 6 hours → Usual dose: 500 mg IV/PO every 6–8 hours → Maximum: 4 g/day Intra-abdominal, Skin, Bone, or Joint Infections → 500 mg orally or IV every 8 hours → Often combined with ceftriaxone or beta-lactam antibiotics Bacterial Vaginosis → 500 mg orally twice daily for 7 days → or 2 g orally as a single dose (higher relapse rate) → or 0.75% gel, 5 g intravaginally once daily for 5 days Trichomoniasis → 2 g orally as a single dose → or 500 mg orally twice daily for 7 days (alternative regimen) Giardiasis → 250 mg orally three times daily for 5–7 days Amebiasis (Intestinal) → 750 mg orally three times daily for 5–10 days CDI – Mild to Moderate (Alternative) → 500 mg orally three times daily for 10–14 days Surgical Prophylaxis → 500 mg IV within 60 minutes before incision → Administer with cefazolin or similar antibiotic → Take with or without food. → Avoid alcohol during and for 48 hours after therapy. → If a dose is missed, take it as soon as remembered. Do not double up. → Complete the entire course even if symptoms improve. Renal/Hepatic Impairment: → Use with caution in severe hepatic dysfunction; dose adjustments may be needed → No routine dose adjustment in ESRD, but administer post-dialysis due to clearance loss

Common side effects: nausea, metallic taste, dry mouth, dizziness, headache, gastrointestinal discomfort Less common effects: peripheral neuropathy, seizures, allergic skin reactions, dark urine, visual disturbances Stop use and seek medical advice if symptoms such as severe dizziness, numbness, or confusion occur. Avoid alcohol intake to prevent disulfiram-like reactions such as nausea, flushing, or cramps.
