Cotellic 20mg is an oral anticancer medication containing cobimetinib, a selective MEK inhibitor used in combination with vemurafenib for treating unresectable or metastatic melanoma with a BRAF V600E or V600K mutation. It is also approved for use in adults with histiocytic neoplasms. Cobimetinib targets the MAPK/ERK signaling pathway, which is often abnormally activated in cancers with BRAF mutations, and provides a precision-targeted option that helps inhibit tumor growth, delay progression, and improve survival outcomes.

This targeted approach is suitable for patients who cannot undergo surgical resection and helps reduce tumor burden and extend progression-free survival. > Histiocytic Neoplasms Cotellic is also approved for treating histiocytic neoplasms, a rare group of blood-derived cancers affecting the immune system. It offers a novel therapeutic option by disrupting MEK-mediated signaling essential for the proliferation of neoplastic histiocytes.
Cobimetinib works by selectively inhibiting MEK1 and MEK2, critical kinases in the RAS/RAF/MEK/ERK pathway that are overactivated in many BRAF-mutant cancers. By shutting down this pathway, Cotellic suppresses abnormal cell proliferation and enhances apoptosis. When combined with vemurafenib (a BRAF inhibitor), it offers dual blockade of the MAPK pathway, improving therapeutic outcomes in BRAF V600-mutated malignancies.
Usual Adult Dose for Melanoma – Metastatic: - 60 mg orally once daily for the first 21 days of each 28-day treatment cycle - Continue until disease progression or intolerable toxicity Usual Adult Dose for Histiocytic Neoplasms: - 60 mg orally once daily for the first 21 days of each 28-day treatment cycle - Continue until disease progression or unacceptable toxicity Comments: - Confirm BRAF V600E or V600K mutation using an FDA-approved test before initiating therapy. - Cotellic must be used in combination with vemurafenib, not as monotherapy. - Administer with or without food. Swallow tablets whole; do not crush or chew. - Monitor liver enzymes, creatine phosphokinase (CPK), cardiac function, and ophthalmologic status during treatment due to known risks.
Store at 20–25°C (68–77°F). Keep in original container, tightly closed. Protect from moisture and light. Do not refrigerate or freeze. Keep out of reach of children.
Common side effects include photosensitivity, rash, diarrhea, nausea, fever, and elevated liver enzymes. Serious risks include retinal detachment, cardiomyopathy, severe muscle toxicity (elevated CPK), and hepatotoxicity. Regular monitoring and patient education on symptoms like blurry vision, fatigue, or muscle pain are essential for safe use. Patients should avoid sun exposure during treatment.
