Efudex 5% Cream - 40g is a topical chemotherapeutic agent containing 5% fluorouracil, formulated for targeted treatment of abnormal skin cell growth. It acts by inhibiting thymidylate synthase, disrupting DNA replication specifically in precancerous and cancerous cells. The cream offers a non-invasive alternative for managing actinic keratosis, superficial basal cell carcinoma, and Bowen’s disease, while also having off-label dermatological applications under specialist supervision.
- AntiCancerDrugs > Actinic Keratosis Efudex 5% Cream is primarily used for treating actinic (solar) keratosis, a precancerous condition resulting from long-term UV exposure. When applied topically, it selectively destroys atypical keratinocytes by interfering with DNA synthesis. The inflammatory skin response it induces—typically redness, scaling, and crusting—leads to lesion removal and regeneration of healthy skin. > Superficial Basal Cell Carcinoma Efudex is indicated for the non-surgical treatment of superficial basal cell carcinoma, especially in patients with multiple lesions or tumors located in surgically sensitive areas. It works by causing localized cytotoxicity and immune-mediated clearance of malignant cells, offering high efficacy with minimal systemic absorption. > Bowen’s Disease Efudex effectively treats Bowen’s disease (squamous cell carcinoma in situ) confined to the epidermis. Applied topically, it eliminates abnormal epithelial cells, preventing progression to invasive carcinoma. This is particularly valuable in cases involving widespread, multifocal, or cosmetically sensitive lesions. > Offlabel Dermatologic Use Beyond its approved indications, Efudex has been used off-label to manage conditions like viral warts, HPV-related skin changes, and keloid scarring. These applications are investigational and should only be undertaken under dermatologic supervision due to variable response and potential for irritation.
- Antimetabolite activity inhibits DNA synthesis in rapidly dividing skin cells - Selective cytotoxic effect on abnormal keratinocytes and malignant epidermal cells - Initiates visible skin response culminating in lesion resolution - Minimally systemic, suitable for localized cutaneous application
1. Actinic Keratosis - Apply 5% cream or solution twice daily to affected areas - Continue application until visible inflammation and lesion erosion occur (typically 2 to 4 weeks) - Healing period may extend 1 to 2 months post-treatment - Application should be done 10 minutes after washing and drying - Avoid eyes, nostrils, and mucous membranes - Wash hands thoroughly after application - Biopsy any non-healing lesions after therapy 2. Superficial Basal Cell Carcinoma - Apply 5% cream twice daily to fully cover the lesion - Treatment duration is typically 3 to 6 weeks; some cases may require up to 12 weeks - Progression of local response includes erythema, scaling, erosion, and reepithelialization - Monitor response and adjust duration as needed under medical guidance 3. Bowen’s Disease - Use 5% fluorouracil cream applied twice daily over lesion surface - Treatment course ranges from 4 to 6 weeks depending on lesion extent - Suitable for patients with contraindications to surgery - Histological follow-up may be necessary to confirm complete response
Store at controlled room temperature below 25°C. Protect from excessive moisture and light. Keep tightly closed and out of reach of children. Do not freeze.
- Common: Local redness, burning, dryness, scaling, crusting - Moderate: Erosions, secondary infection, pruritus, edema - Rare: Allergic contact dermatitis, persistent ulceration, skin discoloration
