Zelboraf 240mg is an oral targeted therapy formulated for patients with BRAF V600 mutation-positive cancers. As a selective BRAF kinase inhibitor, it blocks aberrant MAPK signaling in tumor cells harboring the V600E mutation. It was one of the first drugs approved specifically for BRAF-mutant metastatic melanoma and remains a critical agent in precision oncology. The 240mg film-coated tablets are administered twice daily and often used until progression or the onset of significant toxicity. Zelboraf has also gained off-label recognition for conditions like Erdheim-Chester Disease and rare BRAF-driven solid tumors.

It improves progression-free and overall survival, particularly in patients not previously treated with immune checkpoint inhibitors. > Others (Histiocytosis Erdheim-Chester Disease) Used in adult patients with Erdheim-Chester Disease (ECD) harboring a BRAF V600 mutation. Although a rare hematologic neoplasm, ECD responds well to BRAF inhibition due to its oncogenic driver mutation.
Vemurafenib selectively targets the BRAF V600E kinase, an aberrant driver of MAPK signaling in tumor cells. By inhibiting this pathway, the drug blocks tumor proliferation and promotes apoptosis. It does not affect wild-type BRAF, making it suitable only for patients confirmed to carry the V600E mutation. The therapeutic action results in rapid tumor shrinkage, improved survival rates, and symptom control, especially in aggressive metastatic melanoma.
All patients must be tested and confirmed positive for the BRAF V600E mutation via an FDA-approved diagnostic test before initiating therapy. - Metastatic Melanoma: • 960 mg orally every 12 hours (total 1920 mg/day) • Continue until disease progression or unacceptable toxicity • Tablets may be taken with or without food • Do not crush or split the tablets - Histiocytosis (Erdheim-Chester Disease): • Same dosing: 960 mg orally every 12 hours • Duration based on response and tolerability • Monitor cardiac function, dermatologic adverse events, and liver enzymes [Administration Tips] - Tablets should be swallowed whole with water - If a dose is missed, it should not be taken if more than 4 hours have passed - Dose adjustments may be required based on liver toxicity or QT prolongation
Store at 20°C to 25°C (68°F to 77°F). Excursions permitted between 15°C to 30°C. Keep in original packaging to protect from moisture. Avoid exposure to excessive heat or sunlight.
Common adverse effects include arthralgia, rash, photosensitivity, fatigue, and alopecia. Serious reactions may include cutaneous squamous cell carcinoma (cuSCC), hepatotoxicity, QT prolongation, and hypersensitivity reactions. Dermatologic monitoring is required during therapy. Regular ECGs and liver function tests are recommended. Dose interruption or reduction may be necessary in case of grade 3–4 toxicity.
