Bavencio 200mg is an intravenous immune checkpoint inhibitor classified as an anti-PD-L1 monoclonal antibody. It plays a vital role in the treatment of various advanced cancers by restoring T-cell function and promoting immune-mediated tumor destruction. Bavencio is particularly effective in Merkel Cell Carcinoma (MCC), Urothelial Carcinoma (UC), and Renal Cell Carcinoma (RCC), either as monotherapy or in combination with targeted agents. Its role as a maintenance therapy following chemotherapy makes it a strategic component in long-term cancer control, especially for UC.
- AntiCancer Drugs > Skin Cancer (Merkel Cell Carcinoma) Indicated for adults and adolescents (≥12 years) with metastatic Merkel Cell Carcinoma. As a rare and aggressive skin cancer, MCC responds well to PD-L1 inhibition, and Bavencio offers durable tumor response with manageable toxicity. > Kidney Cancer (Renal Cell Carcinoma) Bavencio is approved for use in combination with axitinib for first-line treatment of advanced or metastatic RCC. This dual approach targets immune escape mechanisms and angiogenesis, showing improved progression-free survival in clinical trials. > Others (Urothelial Carcinoma) Used as maintenance therapy for patients with advanced UC who have not progressed on platinum-containing chemotherapy. It is also indicated for patients with disease recurrence within 12 months of prior platinum-based neoadjuvant or adjuvant therapy.
Avelumab binds to PD-L1, a protein expressed on tumor cells that inhibits T-cell activity by engaging PD-1 receptors. By blocking PD-L1, Bavencio restores the immune system’s ability to recognize and attack cancer cells. Unlike some other PD-1 inhibitors, avelumab retains antibody-dependent cellular cytotoxicity (ADCC), further enhancing its antitumor effect. It is administered biweekly, and treatment continues until disease progression or unacceptable toxicity.
- Merkel Cell Carcinoma (Adults & ≥12 yrs): • 800 mg IV every 2 weeks over 60 minutes • Continue until disease progression or unacceptable toxicity • Premedicate with antihistamine and acetaminophen before first 4 infusions - Urothelial Carcinoma: • 800 mg IV every 2 weeks over 60 minutes • Eligible after stable disease or response to platinum chemo • May also be used for disease progression ≤12 months after platinum-based therapy • Premedication protocol as above - Renal Cell Carcinoma: • Avelumab: 800 mg IV every 2 weeks over 60 minutes • Axitinib: 5 mg orally twice daily • Used as first-line therapy in advanced RCC • Premedication required before initial doses and as needed thereafter [Administration Notes] - Infusions must be administered under supervision - Monitor for immune-related adverse events (pneumonitis, colitis, hepatitis, endocrinopathies) - Consider corticosteroids for grade ≥2 immune reactions - Ensure pre-infusion hydration and vital monitoring during administration
Store vials in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light by keeping in the original carton until use. Avoid shaking or exposure to direct sunlight.
Common side effects include fatigue, infusion-related reactions, musculoskeletal pain, diarrhea, and rash. Serious immune-mediated reactions such as pneumonitis, hepatitis, or colitis may occur. Thyroid dysfunction and adrenal insufficiency are also possible. Infusion reactions are most frequent during initial cycles and may require dose delays or premedication adjustments. Long-term use necessitates regular monitoring of liver enzymes, thyroid function, and renal panels.
