Banocide Forte 100 - 600 Tablets contains diethylcarbamazine citrate (DEC), a piperazine derivative anthelmintic that is the drug of choice for treating lymphatic filariasis and other filarial infections. DEC exerts a microfilaricidal effect by altering the surface membrane and metabolic processes of microfilariae, rendering them vulnerable to host immune clearance. It also has partial macrofilaricidal action, reducing adult worm motility and viability. This dual mechanism interrupts the transmission cycle, reduces disease burden, and alleviates inflammatory symptoms caused by the death of parasites. It is widely used in endemic regions as part of WHO-led Mass Drug Administration (MDA) programs to eliminate lymphatic filariasis, and has applications in treating loiasis, tropical pulmonary eosinophilia, and other filarial diseases. DEC’s rapid clearance of microfilariae makes it effective in breaking transmission chains, while its inclusion in some SSTI-related protocols addresses parasite-associated secondary infections.
- ANTIPARASITICS > Anthelmintics Primary treatment for a broad range of parasitic worm infections, particularly filarial species causing lymphatic disease. - ANTIBIOTICS & INFECTIONS > Skin and Soft Tissue Infections (SSTIs) Occasional off-label use for preventing or managing secondary infections linked to parasitic skin disease.
- Rapidly reduces microfilariae load in blood, limiting transmission to mosquitoes. - Improves symptoms such as lymph node swelling, fever, and lymphangitis. - Decreases risk of chronic complications like elephantiasis by suppressing inflammation and preventing lymphatic damage. - In tropical pulmonary eosinophilia, normalizes eosinophil counts and improves respiratory function within days.
Indication: - Lymphatic filariasis (Wuchereria bancrofti, Brugia malayi, Brugia timori) - Loiasis - Tropical pulmonary eosinophilia - Other filarial infections Adult Dosage & Regimens: - MDA: Single annual dose of 6 mg/kg (with albendazole 400 mg) for 4–6 years in areas without Onchocerca volvulus co-endemicity. - Filarial diseases (individual treatment): Day 1: 50 mg orally after meals Day 2: 50 mg orally three times daily Day 3: 100 mg orally three times daily Days 4–14: Continue 6 mg/kg/day divided into three doses - Tropical pulmonary eosinophilia: 6 mg/kg/day divided three times daily for 14 days - Loiasis: Same initiation as above, then 9 mg/kg/day divided three times daily for days 4–21 Pediatric Dosage: - MDA: ≥2 years – same as adult (6 mg/kg single dose annually) - Individual treatment: Day 1: 1 mg/kg orally after meals Day 2: 1 mg/kg three times daily Day 3: 1–2 mg/kg three times daily Days 4–14: 6 mg/kg/day divided three times daily - Loiasis (pediatric): Same initiation, then 9 mg/kg/day divided three times daily for days 4–21 Method: - Administer orally after meals to minimize gastrointestinal upset. Contraindications & Precautions: - Avoid in onchocerciasis due to severe Mazzotti reaction risk with ocular complications. - Avoid in high Loa loa microfilarial load due to encephalopathy risk. - Exclude children under 2 years, pregnant/breastfeeding women, and those severely ill from MDA campaigns. - Monitor for allergic or inflammatory reactions caused by rapid parasite death.
