Epilepsy

Bipolar Disorder

Adult Dosing Summary
Epilepsy – IR Form: Start with 200 mg twice daily or 100 mg QID (oral suspension). Titrate weekly by 200 mg/day in divided doses (TID or QID). Maintenance: 800–1200 mg/day. Maximum: 1600 mg/day.
ER Form: Use the same total daily dose as IR, divided BID; titrate similarly.
Trigeminal Neuralgia – Start at 100 mg BID or 50 mg QID (oral suspension). Increase by 200 mg/day until pain relief is achieved. Maintenance: 400–800 mg/day. Maximum: 1200 mg/day. Reassess every 3 months for potential taper.
Bipolar Disorder (ER Capsules) – Start at 200 mg BID. Titrate by 200 mg/day to achieve mood stabilization. Maximum: 1600 mg/day.
Pediatric Dosing Summary
<6 Years – 10–20 mg/kg/day divided BID–QID. Titrate weekly as tolerated. Maximum: 35 mg/kg/day.
6–12 Years – Start at 100 mg BID or 100 mg QID (oral suspension). Titrate weekly up to 100 mg/day. Maintenance: 400–800 mg/day. Maximum: 1000 mg/day.
>12 Years – Start at 200 mg BID or 100 mg QID (oral suspension). Titrate weekly by 200 mg/day. Maintenance: 800–1200 mg/day. Maximum: 1000 mg/day (12–15 yrs), 1200 mg/day (>15 yrs).
Additional Considerations
IR to ER conversion: Use the same total daily dose, divided BID.
Suspension use: Same total dose, but increase dosing frequency (TID–QID).
Polytherapy: Add gradually and adjust co-medications (e.g., phenytoin may need upward dose adjustment).
Monitoring: Regular plasma level checks ensure optimal therapeutic effect and minimize side effects.
Not suitable for absence seizures; may worsen generalized seizure types.
Store at room temperature below 25°C in a dry place, protected from direct light and humidity. Keep in original packaging and out of children's reach.
Common side effects include dizziness, drowsiness, nausea, blurred vision, and unsteadiness. Serious reactions may involve skin rashes (including Stevens-Johnson Syndrome), liver enzyme elevation, blood dyscrasias, and hyponatremia. Regular blood tests are recommended during long-term use to monitor liver function, electrolytes, and blood counts.
