Adult Dosing Summary

ER Form: Same total daily dose as IR, divided BID.
Trigeminal Neuralgia – Start at 100 mg BID (or 50 mg QID suspension). Titrate up to 200 mg/day until pain is controlled. Maintenance: 400–800 mg/day; Max: 1200 mg/day. Reassess every 3 months.
Bipolar Disorder / Mania – Start at 200 mg BID. Titrate by 200 mg/day as needed. Max: 1600 mg/day.
Pediatric Dosing Summary
<6 Years – 10–20 mg/kg/day in 2–4 divided doses. Max: 35 mg/kg/day.
6–12 Years – Start at 100 mg BID or QID. Titrate up to 100 mg/week. Maintenance: 400–800 mg/day; Max: 1000 mg/day.
>12 Years – Start at 200 mg BID or 100 mg QID. Titrate weekly by 200 mg/day. Maintenance: 800–1200 mg/day. Max: 1000–1200 mg/day depending on age.
Additional Considerations
Switching IR → ER: Use equivalent total daily dose divided BID.
Suspension: Requires more frequent dosing (TID or QID).

Monitoring: Regular testing of plasma levels, liver enzymes, and CBC is recommended.
Not indicated for absence seizures; may exacerbate them.
Store at room temperature below 25°C, in a dry place away from direct sunlight and moisture. Keep out of reach of children and do not store in bathrooms.
Common side effects include dizziness, drowsiness, nausea, and blurred vision. Serious risks include liver enzyme elevation, leukopenia, hyponatremia, and rare hypersensitivity syndromes such as Stevens-Johnson syndrome. Regular monitoring is essential for long-term users.
