Epilepsy

Carbamazepine stabilizes overexcited nerve cells by blocking voltage-gated sodium channels. This reduces abnormal neuronal firing, which is the underlying cause of seizures and mood fluctuations. It also modifies pain signal transmission, making it effective in treating chronic nerve pain. The controlled-release form helps maintain even plasma drug concentrations, minimizing side effects and ensuring stable clinical effects.
Adult Dosing Summary
Epilepsy (ER Form) – Start at 200–300 mg twice daily. Titrate weekly by 200 mg/day. Maintenance: 800–1200 mg/day. Maximum: 1600 mg/day.
Trigeminal Neuralgia – Start at 100 mg twice daily. Titrate to 200 mg/day or more until effective. Maintenance: 400–800 mg/day. Maximum: 1200 mg/day. Reassess every 3 months.
Bipolar Disorder – Start at 200 mg twice daily. Increase by 200 mg/day as needed. Maximum: 1600 mg/day.
Pediatric Dosing Summary
Under 6 Years – 10–20 mg/kg/day divided 2–4 times daily. Titrate weekly. Maximum: 35 mg/kg/day.
6–12 Years – Start at 100 mg twice daily. Titrate by 100 mg/day weekly. Maintenance: 400–800 mg/day. Maximum: 1000 mg/day.
Over 12 Years – Start at 200 mg twice daily. Increase by 200 mg/day weekly. Maintenance: 800–1200 mg/day. Maximum: 1000 mg/day (12–15 yrs), 1200 mg/day (15+ yrs).
Additional Considerations
When switching from immediate-release to extended-release, maintain the same total daily dose in two divided doses.
When switching to suspension form, increase frequency to three or four times daily.
In polytherapy, titrate carbamazepine slowly while monitoring for drug interactions.
Regular plasma level monitoring can aid in optimizing efficacy and minimizing side effects.
Not recommended for absence seizures, as it may worsen them.
Store at room temperature below 25°C in a dry place away from moisture and sunlight. Keep the tablets in their original packaging and out of reach of children. Do not store near sinks or humid environments.
Common side effects may include dizziness, drowsiness, nausea, and visual disturbances. Some users may experience unsteady gait or coordination problems.
In rare cases, serious reactions such as liver enzyme elevation, hyponatremia, skin hypersensitivity (Stevens-Johnson syndrome), or blood disorders can occur.
Regular blood tests for liver function, sodium levels, and complete blood counts are advised, especially during the early phase of treatment or dose changes.
