Mazetol 400 is a generic extended-release formulation of carbamazepine 400mg. It is primarily used to manage epilepsy, bipolar disorder, and various types of neuropathic pain. The sustained-release design helps maintain stable plasma drug levels, minimizing peaks and troughs in symptom control while enhancing patient compliance, particularly for long-term therapy.

Mazetol 400 acts as a mood stabilizer in bipolar disorder, especially useful for acute mania and preventing recurrence. It is often used when lithium is not well tolerated.

Carbamazepine works by blocking sodium channels in the central nervous system, reducing the excitability of overactive neurons. This leads to decreased seizure frequency, mood stabilization, and significant relief from neuropathic pain. Its extended-release property provides smoother blood levels and reduces the need for multiple daily dosing.
Adult Dosing
• Epilepsy
→ Start: 200 mg BID (or 100 mg QID suspension)
→ Increase weekly by up to 200 mg/day in divided doses
→ Maintenance: 800–1200 mg/day
→ Maximum: 1600 mg/day
• Trigeminal Neuralgia
→ Start: 100 mg BID (or 50 mg QID suspension)
→ Increase until pain is controlled (maximum: 1200 mg/day)
→ Reassess every 3 months
• Bipolar Disorder
→ Start: 200 mg BID
→ Titrate by 200 mg/day based on clinical response
→ Maximum: 1600 mg/day
Pediatric Dosing
• <6 Years
→ 10–20 mg/kg/day divided BID–QID
→ Maximum: 35 mg/kg/day
• 6–12 Years
→ Start: 100 mg BID
→ Increase weekly by up to 100 mg/day
→ Maintenance: 400–800 mg/day
→ Maximum: 1000 mg/day
• >12 Years
→ Start: 200 mg BID
→ Increase weekly by up to 200 mg/day
→ Maintenance: 800–1200 mg/day
→ Maximum: 1000–1200 mg/day
Additional Considerations
→ Use the same total daily dose when switching from IR → ER (divide BID)
→ Do not abruptly switch from tablets to suspension without adjusting frequency
→ When used in polytherapy, titrate carefully and monitor for drug interactions
→ Not suitable for absence seizures; may exacerbate them
