Tonact 40 is a generic formulation of atorvastatin 40mg, a high-intensity statin therapy recommended for patients requiring aggressive lipid control. Atorvastatin belongs to the HMG-CoA reductase inhibitor class and is extensively used to lower elevated cholesterol levels and reduce cardiovascular risk. Atorvastatin acts by competitively inhibiting HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis. This results in upregulation of hepatic LDL receptors, leading to increased clearance of LDL particles from the blood. The 40mg dose provides high-intensity LDL-C reduction (>45%) and is effective for managing complex lipid disorders and preventing progression of atherosclerosis.
General Hyperlipidemia / HeFH / Type III Dyslipidemia / Hypertriglyceridemia: - Starting dose: 10–20 mg once daily - Maintenance dose: 10–80 mg once daily - Consider initiating at 40 mg/day in patients requiring >45% LDL-C reduction - Must be combined with a lipid-lowering diet and lifestyle changes Homozygous Familial Hypercholesterolemia (HoFH): - Adult dose: 10–80 mg once daily - Pediatric (≥10 years): 10–80 mg once daily - May be used in conjunction with LDL apheresis where available Heterozygous Familial Hypercholesterolemia (HeFH): - Adults: 10–80 mg once daily - Children (≥10 years): 10–20 mg once daily - Adjust dosage every 4–6 weeks based on lipid response Cardiovascular Disease Prevention (Primary & Secondary): - Initial dose: 10–20 mg once daily - Maintenance dose: 10–80 mg once daily - Beneficial in high-risk patients regardless of baseline LDL-C - Particularly effective in patients with diabetes, metabolic syndrome, or pre-existing ASCVD
