Hycamtin 1 contains topotecan 1mg, an oral formulation of a topoisomerase I inhibitor used primarily for the treatment of relapsed or refractory solid tumors. It acts by stabilizing the topoisomerase I-DNA complex, resulting in DNA strand breaks and inhibition of replication in rapidly proliferating cancer cells. Due to its oral bioavailability and well-documented efficacy, Hycamtin is a preferred option for platinum-resistant ovarian cancer and relapsed small cell lung cancer (SCLC). Its flexible administration routes (oral and IV) offer convenience in palliative and salvage chemotherapy settings.
AntiCancer Drugs > Platinum-Resistant Ovarian Cancer
Used as monotherapy in patients who failed prior platinum-based regimens. It is recommended for those whose disease recurs within 6 months after initial treatment and has shown delayed but durable responses over multiple cycles.
Small Cell Lung Cancer (SCLC)
Indicated for relapsed cases after failure of first-line chemotherapy (e.g., cisplatin + etoposide). Hycamtin can be administered orally or intravenously, and offers both symptomatic relief and progression delay in sensitive relapses (≥45–90 days after first-line therapy).
Cervical Cancer (Metastatic or Recurrent)
Used in combination with cisplatin as a second-line regimen for persistent, recurrent, or metastatic cervical carcinoma. It may provide modest survival benefit in non-curative clinical contexts.
Breast, Brain, and Endometrial Cancers
Explored in experimental regimens or clinical trials as part of combination chemotherapy in select tumor types, often in conjunction with anti-angiogenic agents such as bevacizumab.
Inhibits topoisomerase I enzyme, preventing re-ligation of single-strand DNA breaks
Induces double-strand DNA damage during replication, leading to apoptosis
Most effective in rapidly dividing tumor cells with high replication stress
Can be administered orally or intravenously, depending on clinical need
Ovarian Cancer (IV)
→ 1.5 mg/m² IV over 30 min once daily for 5 days
→ Repeat every 21 days
→ Recommend minimum 4 cycles due to delayed median response (9–12 weeks)
→ Maximum single dose: 4 mg
Use: For metastatic or recurrent ovarian cancer after prior chemotherapy failure
Cervical Cancer (IV, with Cisplatin)
→ 0.75 mg/m² IV over 30 min on Days 1, 2, and 3 of each 21-day cycle
→ Cisplatin 50 mg/m² IV on Day 1 of each cycle
→ Max topotecan dose: 4 mg
Use: For advanced, recurrent, or persistent cervical carcinoma unsuitable for curative therapy
Small Cell Lung Cancer (SCLC)
→ IV: 1.5 mg/m² IV over 30 min once daily for 5 days every 21 days
→ Oral: 2.3 mg/m² orally once daily for 5 days per 21-day cycle (round to nearest 0.25 mg)
→ Recommend at least 4 cycles; median time to response: 5–7 weeks
→ Do not replace oral doses if vomiting occurs
→ Max IV dose: 4 mg
Use: For relapsed SCLC after initial response to first-line chemotherapy, based on time to relapse (≥45–90 days)
