Bdpoma 2 is a 2 mg formulation of pomalidomide, a third-generation immunomodulatory drug (IMiD) developed for advanced hematologic malignancies. It is primarily used in combination with dexamethasone for patients with relapsed or refractory multiple myeloma (RRMM) who have failed at least two prior lines of therapy. Pomalidomide exerts anti-inflammatory, anti-angiogenic, and direct antineoplastic activities, providing improved disease control and a survival benefit in later treatment stages.

Pomide 2 is indicated for patients with relapsed or treatment-resistant multiple myeloma after failure of lenalidomide and proteasome inhibitors. Administered with dexamethasone, it enhances both immunomodulatory and cytotoxic effects, helping to prolong progression-free survival and overall survival in high-risk populations.
> Triple-Class Refractory Multiple Myeloma
Pomide 2 is also used as salvage therapy in patients refractory to immunomodulators (IMiDs), proteasome inhibitors (PIs), and anti-CD38 monoclonal antibodies. It is frequently combined with novel agents such as daratumumab or carfilzomib in clinical protocols, maintaining its role as a viable last-line defense in aggressive disease.
> Kaposi’s Sarcoma
Pomide 2 is approved for AIDS-related Kaposi’s sarcoma after HAART failure, as well as for HIV-negative patients with Kaposi’s lesions. Its potent anti-angiogenic activity is critical in suppressing lesion progression and limiting systemic involvement.
Pomalidomide modulates the immune response by enhancing T-cell and natural killer (NK) cell activation. It inhibits angiogenesis and interferes with tumor cell proliferation, which collectively contribute to reducing tumor burden. These mechanisms also help restore hematologic function in refractory multiple myeloma cases. The drug is particularly effective in patients who have exhausted prior treatment options, maintaining disease control with manageable toxicity.
For multiple myeloma, the usual adult dose is 4 mg orally once daily on Days 1 through 21 of each 28-day cycle. It must always be given in combination with dexamethasone, and treatment should be continued until disease progression or the development of unacceptable toxicity.
For Kaposi’s sarcoma, the recommended adult dose is 5 mg orally once daily on Days 1 through 21 of each 28-day cycle. Therapy should be continued until disease progression or unacceptable toxicity. In AIDS-related KS, patients must continue HAART throughout pomalidomide therapy.
Additional Instructions:
The medication should be taken at the same time each day, with water, on an empty stomach. Pregnancy must be strictly avoided due to the drug’s high teratogenicity. Routine monitoring of blood counts and liver function is essential. Prophylactic anticoagulation should be considered for patients at increased risk of venous thromboembolism.
Store at room temperature below 25°C. Keep in original blister packaging away from moisture and light. Handle with care; avoid contact with broken capsules.
Common side effects include neutropenia, thrombocytopenia, fatigue, constipation, and upper respiratory infections. Serious risks include venous thromboembolism, hepatotoxicity, and secondary malignancies. Regular monitoring is critical for early detection and management of adverse effects.
