Lenmid 25 is a high-strength oral formulation of lenalidomide, an immunomodulatory agent classified under antineoplastic drugs. This 25 mg dosage is primarily used as the starting dose in patients with multiple myeloma in combination with dexamethasone, or in cases requiring aggressive disease control such as relapsed mantle cell lymphoma.
Its mechanism targets the cereblon protein complex, resulting in the degradation of transcription factors essential for tumor growth, while simultaneously enhancing immune-mediated cytotoxicity.

AntiCancer Drugs > Multiple Myeloma
Lenmid 25 is frequently initiated as part of first-line combination therapy for multiple myeloma, especially with dexamethasone. It is essential in both newly diagnosed and relapsed/refractory cases, where strong immunomodulatory activity is required. The 25 mg strength ensures optimal suppression of malignant plasma cells during the early treatment phases.
> Myelodysplastic Syndrome (MDS)
While lower strengths are more commonly used, Lenmid 25 may be applicable in dose-escalation strategies or in selected patients showing resistance to standard 10 mg doses. It offers potential for hematologic improvement and transfusion independence in 5q deletion-associated MDS.
> Lymphoma
Lenmid 25 supports treatment in patients with relapsed or refractory mantle cell lymphoma and is also used off-label in indolent lymphomas such as follicular and marginal zone subtypes. Its mechanism enhances the effectiveness of rituximab-based regimens, providing a chemo-free treatment alternative in certain populations.

Emerging clinical protocols incorporate lenalidomide in chronic lymphocytic leukemia (CLL) and other B-cell malignancies, particularly as part of novel immune-based combinations or maintenance therapies.

1. Multiple Myeloma (MM)
With dexamethasone: 25 mg orally once daily on Days 1–21 of a 28-day cycle. Continue until progression or intolerable adverse events.
Maintenance after auto-HSCT: Begin with 10 mg daily on Days 1–28 for 3 cycles. If well tolerated, escalate to 15 mg daily and maintain until progression or unacceptable toxicity.
Notes:
Thromboprophylaxis is recommended during treatment.
Monitor CBC, renal, and liver function regularly.
Avoid use in pregnancy due to severe teratogenicity.
Adjust dose in cases of renal impairment.
2. Myelodysplastic Syndromes (MDS)
10 mg once daily, adjusted according to hematologic response.
May consider dose escalation if response is inadequate and treatment is tolerated.
Indicated for transfusion-dependent anemia in 5q deletion subtype MDS.
3. Lymphoma
Follicular / Marginal Zone Lymphoma (FL/MZL): 20 mg once daily on Days 1–21 of each 28-day cycle, for up to 12 cycles, in combination with rituximab.
Mantle Cell Lymphoma (MCL): 25 mg once daily on Days 1–21 of a 28-day cycle. Continue until progression or toxicity.
Uses:
For MCL after ≥2 previous therapies.
Combined with rituximab for previously treated indolent lymphomas.
Store Lenmid 25 capsules at room temperature (15–25°C) in a tightly sealed container. Protect from light and moisture. Due to its teratogenic risk, capsules should not be broken or crushed. Follow specialized handling and disposal protocols for hazardous agents.
