Lenalid 15 is an oral immunomodulatory anticancer agent containing 15 mg of lenalidomide. It is structurally related to thalidomide but demonstrates enhanced potency and a broader therapeutic window.
This strength is primarily intended for escalation in maintenance therapy or for advanced stages requiring intensified dosing.
Lenalid 15 is utilized in the management of various hematologic malignancies, particularly where durable immune modulation and selective cytotoxicity are essential.
Its efficacy is derived from modulating the cereblon pathway, altering the tumor microenvironment, and enhancing T-cell and NK-cell responses against malignant cells.
AntiCancer Drugs > Multiple Myeloma
Lenalid 15 is a key therapeutic option for patients undergoing long-term treatment of multiple myeloma, especially in the post-transplant setting. It is commonly combined with dexamethasone for initial therapy or included in triplet regimens. The 15 mg strength is often employed during maintenance phases, sustaining disease control with tolerable toxicity.
> Myelodysplastic Syndrome (MDS)
Indicated for transfusion-dependent anemia in patients with low to intermediate-1 risk MDS associated with 5q deletion. Lenalid 15 promotes erythropoiesis, helps normalize hematologic profiles, and contributes to transfusion independence in a substantial proportion of treated patients.
> Lymphoma
Lenalid 15 supports therapy in indolent lymphomas such as follicular and marginal zone lymphoma, when combined with rituximab. In mantle cell lymphoma, it serves as a later-line therapy, offering cytostatic control following relapse or failure of prior regimens.
> Other Hematologic Cancers
Off-label applications include chronic lymphocytic leukemia (CLL) and other lymphoproliferative disorders. The immunomodulatory properties of Lenalid 15 are particularly beneficial in patients unresponsive to cytotoxic chemotherapy.

1. Multiple Myeloma (MM)
With dexamethasone: 25 mg orally once daily on Days 1–21 of a 28-day cycle. Continue until progression or unacceptable toxicity.
Maintenance after auto-HSCT: Begin with 10 mg daily for 3 cycles (Days 1–28), then escalate to 15 mg once daily if tolerated. Continue until disease progression.
Notes:
Monitor blood counts, renal function, and thrombotic risk.
Use stem cell mobilization protocols before extended exposure.
Consider anticoagulation during therapy.
2. Myelodysplastic Syndromes (MDS)
10 mg orally once daily.
Titrate based on cytogenetic response and tolerance.
Continue until clinical benefit or toxicity.
Use:
For anemia in patients with 5q deletion subtype MDS, with or without other cytogenetic abnormalities.
3. Lymphoma
Follicular / Marginal Zone Lymphoma (FL/MZL): 20 mg daily on Days 1–21 of a 28-day cycle, up to 12 cycles, administered with rituximab.
Mantle Cell Lymphoma (MCL): 25 mg daily on Days 1–21 of a 28-day cycle. Continue until disease progression or unacceptable side effects.
Uses:
In relapsed/refractory (R/R) MCL after two or more prior therapies.
In R/R FL or MZL in combination with anti-CD20 therapy.
Store Lenalid 15 capsules at 15–25°C in a dry place away from direct sunlight.
Maintain in original packaging to protect from moisture.
Handle with care due to teratogenicity—do not crush or open capsules.
Follow hazardous drug disposal protocols for unused or expired product.
Common: fatigue, neutropenia, thrombocytopenia, rash, constipation
Serious: venous thromboembolism, tumor flare, hepatotoxicity
Long-term: risk of secondary malignancies, infection, hypothyroidism
