Pomide 4 is an oral formulation of pomalidomide, a third-generation immunomodulatory drug (IMiD) developed for the treatment of relapsed or refractory multiple myeloma (RRMM). Structurally related to thalidomide and lenalidomide, it exhibits enhanced potency in both immunomodulation and direct antitumor activity. The drug works by stimulating T-cell and natural killer (NK) cell function, inhibiting angiogenesis, and promoting apoptosis of malignant cells. Pomide 4 is particularly beneficial for patients who have exhausted prior therapies and require a robust, well-tolerated treatment option.

Pomide 4 is indicated in combination with dexamethasone for adult patients with multiple myeloma who have failed at least two prior therapies, including lenalidomide and a proteasome inhibitor. It improves progression-free survival, increases major response rates, and delays disease advancement, even in high-risk cytogenetic subgroups.
> Kaposi’s Sarcoma (KS), both AIDS-related and HIV-negative
Pomalidomide has shown clinical benefit in treating Kaposi’s sarcoma regardless of HIV status. In AIDS-related KS, it is an option following HAART failure. By inhibiting tumor angiogenesis and modulating immune responses, it offers a non-cytotoxic alternative for long-term disease control.
> Off-label Use in Lymphomas or Leukemias
Pomalidomide is under investigation in T-cell lymphomas and chronic lymphocytic leukemia (CLL), where it is combined with targeted or immunomodulatory agents. These uses remain research-based and are not yet part of standard therapy.
Pomide 4 stimulates T-cell proliferation and cytokine production, enhances NK cell-mediated cytotoxicity, suppresses tumor-induced angiogenesis, and induces apoptosis of myeloma and Kaposi sarcoma cells.
multiple myeloma
the recommended adult dose is 4 mg orally once daily on Days 1–21 of each 28-day cycle, administered with dexamethasone. Therapy is continued until disease progression or unacceptable toxicity.
For Kaposi’s sarcoma, the dose is 5 mg orally once daily on Days 1–21 of each 28-day cycle, continued until progression or unacceptable toxicity.
Additional Instructions:
Take on an empty stomach at the same time each day. Do not take a missed dose if more than 12 hours have passed; resume at the next scheduled dose. Effective contraception is required for both men and women due to teratogenic risk. Dexamethasone dosing must be managed separately based on the patient profile.
Store below 25°C in a dry place, away from direct light. Keep in the original packaging until use. Ensure the product is out of reach of children and properly disposed of after expiration.
Common adverse events include fatigue, neutropenia, anemia, thrombocytopenia, constipation, and dyspnea. Serious risks involve venous thromboembolism, hepatotoxicity, and severe birth defects. Regular monitoring of complete blood counts and liver function is essential. Prophylactic anticoagulation may be recommended depending on individual thrombotic risk.
