Giolen 15 is a high-strength oral anticancer capsule containing 15 mg of lenalidomide, a potent immunomodulatory agent derived from thalidomide.
It demonstrates a multifaceted mechanism of action, including inhibition of angiogenesis, induction of apoptosis in malignant cells, and stimulation of immune-mediated cytotoxicity.
Its primary role lies in treating hematologic cancers, where it is used both in combination therapies and as maintenance treatment to prolong survival and delay progression.
The 15 mg dosage allows for dose escalation in maintenance regimens or for tailored treatment protocols.

Giolen 15 is essential in the management of multiple myeloma, particularly in the maintenance setting following autologous hematopoietic stem cell transplantation. It modulates the immune environment and inhibits tumor-supportive cytokines, contributing to durable remission. It is also part of upfront therapy when used in conjunction with dexamethasone or other proteasome inhibitors.
Myelodysplastic Syndrome (MDS)
For patients with 5q deletion-associated MDS, Giolen 15 improves erythroid lineage recovery and significantly reduces the need for transfusions. It enhances bone marrow function, slows disease progression, and is FDA-approved for this subtype.
Lymphoma
Giolen 15 plays a therapeutic role in mantle cell lymphoma (MCL) as a second-line or later-line treatment. It is also combined with monoclonal antibodies like rituximab for follicular and marginal zone lymphomas, showing enhanced immune-mediated anti-lymphoma effects.
Other Hematologic Malignancies
Off-label usage in chronic lymphocytic leukemia (CLL) and other lymphoproliferative disorders has demonstrated promising immune activation and cytostatic effects, offering an alternative for patients unfit for conventional chemotherapy.
Multiple Myeloma (MM)
• In combination with dexamethasone: 25 mg orally once daily on Days 1–21 of a 28-day cycle
Continue until disease progression or unacceptable toxicity
• Maintenance post-autologous stem cell transplant: Start with 10 mg daily for 3 cycles, then escalate to 15 mg once daily on Days 1–28 per cycle
Notes:
Monitor complete blood count and renal function regularly
Stem cell collection should occur before prolonged lenalidomide exposure
Thromboprophylaxis recommended during treatment
Myelodysplastic Syndromes (MDS)
• 10 mg orally once daily
• Adjust based on hematologic and cytogenetic response
• Continue as long as clinical benefit persists
Use:
For anemia due to MDS with 5q deletion with/without additional cytogenetic abnormalities
Lymphoma
• Follicular Lymphoma / Marginal Zone Lymphoma: 20 mg daily on Days 1–21 of a 28-day cycle for up to 12 cycles, with rituximab
• Mantle Cell Lymphoma: 25 mg daily on Days 1–21 of a 28-day cycle
Continue until progression or toxicity
Uses:
For MCL after ≥2 prior therapies including bortezomib
In combination with rituximab for FL or MZL
Storage Instructions
Store Giolen 15 capsules at controlled room temperature (15–25°C). Keep in original blister packaging away from light and moisture.
Handling Precautions
Due to its teratogenic potential, strict handling precautions must be followed. Do not open or crush capsules.
Disposal
Ensure disposal through pharmacy-directed hazardous drug procedures.
Frequent: Neutropenia, anemia, fatigue, diarrhea, rash
Serious: Deep vein thrombosis, tumor lysis syndrome, hepatotoxicity
Long-term: Secondary primary malignancies, infections, thyroid dysfunction
