Nizral Cream 30g contains ketoconazole, a potent broad-spectrum imidazole antifungal agent developed for topical treatment of superficial mycoses. It works by inhibiting ergosterol synthesis in fungal cell membranes, leading to membrane instability, leakage of intracellular components, and eventual fungal cell death. This makes it highly effective for athlete’s foot, ringworm, cutaneous candidiasis, and tinea versicolor. It is also widely used for seborrheic dermatitis, including scalp involvement, where it helps control Malassezia yeast overgrowth, reduce inflammation, and prevent recurrence. Off-label, it is sometimes included in post-acne care regimens to manage seborrheic flare-ups and fungal-related skin irritation, promoting a smoother and clearer complexion.

Ketoconazole selectively inhibits the fungal cytochrome P450 enzyme 14α-demethylase, a critical catalyst in ergosterol biosynthesis. Without ergosterol, fungal cell membranes lose structural integrity, disrupting nutrient transport and metabolic functions. In addition to its antifungal effect, ketoconazole possesses mild anti-inflammatory activity, making it suitable for inflamed or irritated skin conditions such as seborrheic dermatitis.
For topical infections (athlete’s foot, ringworm, candidiasis, tinea versicolor, seborrheic dermatitis): Apply a thin layer to the affected area and 2 cm beyond the lesion’s edge once or twice daily, depending on severity, continuing for several days after symptoms resolve to prevent relapse. For seborrheic dermatitis: Apply to affected skin twice daily until remission, then reduce frequency for maintenance (e.g., twice weekly). For systemic fungal infections (oral ketoconazole, reserved for cases when other therapies are not available or tolerated): - Blastomycosis, Chromomycosis, Coccidioidomycosis, Histoplasmosis, Paracoccidioidomycosis: Adults – 200 mg once daily; may increase to 400 mg once daily if response is inadequate; treat for 6 months. Pediatrics ≥2 years – 3.3 to 6.6 mg/kg once daily for 6 months. Important: Not recommended for fungal meningitis due to poor cerebrospinal fluid penetration. IDSA guidelines recommend amphotericin B or itraconazole as first-line therapies for blastomycosis and histoplasmosis, reserving ketoconazole for cases where benefits outweigh hepatotoxicity risks. General advice: Clean and dry the affected area thoroughly before application. Avoid contact with eyes, mucous membranes, or open wounds. If a dose is missed, apply promptly unless it is almost time for the next scheduled application.
Store at room temperature (below 25°C), away from direct sunlight and moisture. Keep the cap tightly closed after use. Do not freeze. Keep out of reach of children.
Common: Mild itching, redness, stinging, or dryness at the application site. Rare but serious: Allergic contact dermatitis, swelling, blistering. Oral ketoconazole may cause hepatotoxicity, adrenal suppression, gastrointestinal discomfort, and QT prolongation—requires close medical supervision. Discontinue use and seek medical attention if severe irritation or allergic symptoms occur.
