Terbinaforce 250 - 14 Tablets contains terbinafine, an allylamine-class antifungal agent known for its potent and selective action against dermatophytes and certain yeast infections. It works by inhibiting squalene epoxidase, an essential enzyme in the ergosterol biosynthesis pathway, leading to toxic squalene accumulation within fungal cells and subsequent cell death. Terbinafine offers excellent penetration into keratinized tissues, ensuring deep eradication of fungi in skin, hair, and nails, with a low recurrence rate. It is a first-line systemic option for onychomycosis and severe tinea infections and can also be used off-label in dermatology to help control secondary fungal involvement in post-acne recovery.

Terbinafine selectively inhibits the enzyme squalene epoxidase in the fungal cell membrane synthesis pathway. This blockage prevents the formation of ergosterol while causing intracellular accumulation of squalene, which is toxic to fungi. It is fungicidal against dermatophytes and some molds, and fungistatic or fungicidal against certain yeasts. Its lipophilic nature allows rapid distribution into skin, nails, and hair, maintaining effective concentrations even after therapy completion.
Onychomycosis – Fingernail: 250 mg orally once daily for 6 weeks. Confirm fungal infection via laboratory tests prior to initiation. Monitor liver function before and during therapy. Optimal nail clearance may take months after treatment due to slow nail regrowth. Onychomycosis – Toenail: 250 mg orally once daily for 12 weeks. Same precautions as for fingernail infections. Tinea Capitis: Adults – 250 mg orally once daily for 6 weeks. Pediatrics (≥4 years): - <25 kg: 125 mg once daily - 25–35 kg: 187.5 mg once daily - >35 kg: 250 mg once daily Alternative tablet dosing: 5 mg/kg/day – 10–20 kg: 62.5 mg; 21–40 kg: 125 mg; ≥41 kg: 250 mg. Duration: Trichophyton tonsurans – 2–4 weeks; Microsporum canis – longer courses. Tinea Corporis: 250 mg orally once daily for 2–4 weeks. Tinea Cruris: 250 mg orally once daily for 2–4 weeks. Tinea Pedis: 250 mg orally once daily for 2–6 weeks. General advice: Always assess for chronic or active liver disease before starting therapy. Take with or without food. Adherence to full treatment duration is essential to minimize relapse risk.

Common: Gastrointestinal discomfort, taste disturbances, headache, mild skin rashes. Less common but serious: Hepatotoxicity, severe skin reactions (e.g., Stevens–Johnson syndrome), neutropenia.
