Terbinaforce 500 - 21 Tablets contains terbinafine, a highly potent allylamine antifungal medication used to treat a broad spectrum of superficial and some deep fungal infections. By inhibiting the fungal-specific enzyme squalene epoxidase, terbinafine disrupts ergosterol biosynthesis, an essential component of fungal cell membranes, leading to toxic squalene accumulation and fungal cell death. Its strong lipophilicity allows rapid penetration into keratin-rich tissues such as skin, nails, and hair, ensuring effective and sustained antifungal activity. Clinically, it is a first-line choice for onychomycosis and severe dermatophyte infections, with a low relapse rate when used appropriately. It is also occasionally used off-label to help prevent secondary fungal colonization in post-acne care, particularly in patients prone to seborrheic or fungal folliculitis.

Terbinafine is fungicidal against dermatophytes and certain molds, and fungistatic or fungicidal against some yeasts. Its mechanism of action involves blocking squalene epoxidase, halting ergosterol production while allowing intracellular squalene to accumulate to toxic levels, leading to fungal death. Because of its keratin-binding properties, terbinafine remains in skin, hair, and nails for weeks after completion of therapy, which contributes to its effectiveness and low recurrence rates.
Onychomycosis – Fingernail: 250 mg orally once daily for 6 weeks. Confirm fungal infection via laboratory testing before starting and evaluate liver function prior to and during treatment. Clinical resolution may take several months after therapy ends due to nail regrowth time. Onychomycosis – Toenail: 250 mg orally once daily for 12 weeks. Follow the same monitoring protocols as for fingernails. Tinea Capitis: Adults – 250 mg orally once daily for 6 weeks. Pediatrics (≥4 years): - <25 kg: 125 mg once daily - 25–35 kg: 187.5 mg once daily - >35 kg: 250 mg once daily Alternative dosing: 5 mg/kg/day – 10–20 kg: 62.5 mg; 21–40 kg: 125 mg; ≥41 kg: 250 mg. Duration: Trichophyton tonsurans – 2–4 weeks; Microsporum canis – longer treatment may be required. Tinea Corporis: 250 mg orally once daily for 2–4 weeks. Tinea Cruris: 250 mg orally once daily for 2–4 weeks. Tinea Pedis: 250 mg orally once daily for 2–6 weeks. General advice: Tablets should be swallowed whole with water, with or without food. Complete the full course even if symptoms improve early to avoid recurrence. Liver function tests are advised for prolonged use or in patients with hepatic risk factors.
Store at room temperature (below 25°C), away from direct sunlight and moisture. Keep in original blister packaging until use. Keep out of reach of children.
Common: Gastrointestinal upset, headache, altered taste perception, mild skin rash. Less common but serious: Hepatotoxicity, severe cutaneous adverse reactions (e.g., Stevens–Johnson syndrome), hematologic abnormalities. Seek immediate medical attention if signs of liver injury (jaundice, dark urine) or severe skin reactions develop.
