Syntran SB 50 contains Itraconazole 50mg, a broad-spectrum triazole antifungal that inhibits the synthesis of ergosterol, a vital component of fungal cell membranes. By disrupting membrane integrity, itraconazole effectively halts fungal cell growth and replication.
With systemic activity, Syntran SB 50 is indicated for both superficial and deep fungal infections. Its strong affinity for keratinous tissues such as skin and nails enables effective treatment of onychomycosis and dermatophytosis.
Itraconazole also achieves therapeutic concentrations in the lungs, mucosal tissues, and certain CNS sites, making it valuable in managing severe systemic mycoses such as aspergillosis and cryptococcosis. Furthermore, it is used prophylactically in immunocompromised patients, including those with hematological malignancies or post-transplant, to reduce the risk of invasive fungal infections.

Key Benefits / Mechanism
Itraconazole inhibits fungal cytochrome P450-dependent lanosterol 14-α-demethylase, impairing ergosterol synthesis and increasing membrane permeability. This disruption causes fungal cell dysfunction and ultimately death.
Its lipophilic nature promotes strong tissue penetration, especially in skin, nails, lungs, and mucosal surfaces. At lower concentrations, itraconazole acts as fungistatic, while at higher systemic exposures it becomes fungicidal.
It is not associated with dependence and is suitable for long-term prophylaxis in high-risk populations. Therapeutic drug monitoring may be required during prolonged use to ensure effective plasma concentrations and minimize resistance or toxicity.
Blastomycosis: 200 mg three times daily for 3 days, then 200 mg once daily. Severe/CNS cases: initial IV amphotericin B, then oral itraconazole 200 mg twice–three times daily for ≥12 months.
• Histoplasmosis: 200 mg three times daily for 3 days, then 200 mg once–twice daily. Acute pulmonary: 6–12 weeks; Disseminated/CNS: ≥12 months.
• Aspergillosis: 200 mg three times daily × 3 days, then 200 mg once–twice daily. Commonly used as salvage therapy or for ABPA.
• Oral Thrush: 200 mg once daily for 1–2 weeks; refractory cases 100 mg twice daily for up to 28 days.
• Esophageal Candidiasis: 100–200 mg once daily for ≥3 weeks and at least 2 additional weeks after symptom resolution.
• Onychomycosis: Toenails – 200 mg once daily for 12 weeks. Fingernails – 200 mg twice daily for 1 week, repeat after 3-week interval × 2 cycles.
• Coccidioidomycosis: 200 mg twice daily for pulmonary/bone infections; CNS involvement may require lifelong therapy.
• Sporotrichosis: Cutaneous – 200 mg once daily for 3–6 months. Disseminated/CNS – 200 mg twice daily for ≥12 months.
• Cryptococcosis (non-CNS): 200 mg twice daily for 6–12 months.
• Cryptococcal Meningitis (immunosuppressed): 200 mg twice daily for ≥8 weeks (consolidation phase).
• Vaginal Candidiasis: 200 mg once daily for 3–7 days.
• Microsporidiosis: 400 mg once daily, combined with albendazole for disseminated forms.
• Paracoccidioidomycosis: 200 mg once daily for 9–18 months.
Pediatrics
• Blastomycosis: 10 mg/kg once daily for 6–12 months (Max: 400 mg/day); severe cases require amphotericin B induction.
• Histoplasmosis: 5–10 mg/kg/day divided twice daily (Max: 400 mg/day). Disseminated and CNS disease requires extended therapy.
• HIV-Infected Children: 2–5 mg/kg three times daily × 3 days, then twice daily for 12 months. Secondary prophylaxis: 5–10 mg/kg once daily.
• Oral Thrush (fluconazole-resistant): 2.5 mg/kg twice daily for 7–14 days (Max: 400 mg/day).
• Esophageal Candidiasis: 2.5 mg/kg twice daily for ≥3 weeks and 2 weeks beyond symptom clearance.
• Coccidioidomycosis: 2–5 mg/kg three times daily × 3 days, then twice daily (Max: 200 mg/dose). Adolescents: 200 mg twice–three times daily.
• Cryptococcosis: 5 mg/kg once daily for ≥1 year (Max: 200 mg/dose).
• Cryptococcal Meningitis: Loading – 2.5–5 mg/kg three times daily × 3 days; Maintenance – 5–10 mg/kg/day for ≥8 weeks (Max: 400 mg/day).
• Vaginal Candidiasis (adolescents): 200 mg once daily for 3–7 days.
• Microsporidiosis: 400 mg once daily (adolescents), co-administered with albendazole.
• Sporotrichosis: 6–10 mg/kg once daily (Max: 400 mg/day); disseminated cases require the same extended regimen.
Administration Guidance
Take itraconazole immediately after a full meal to maximize absorption. Avoid concomitant use with antacids, H2 blockers, or proton pump inhibitors unless clinically necessary and spaced appropriately. If a dose is missed, take as soon as remembered unless near the next scheduled dose—do not double. Monitor liver enzymes during prolonged therapy. Use cautiously in patients with hepatic impairment. Contraindicated in patients with congestive heart failure or ventricular dysfunction.
Store in a cool, dry place below 25°C.

Keep out of reach of children.
Common: nausea, vomiting, abdominal discomfort, headache, dizziness, and rash.
Serious: hepatotoxicity, congestive heart failure, peripheral neuropathy, and QT prolongation.
Rare but severe: Stevens-Johnson syndrome has been reported.
Monitoring: periodic liver function testing is strongly recommended during prolonged therapy.
Seek care: stop use and seek medical attention if jaundice, severe fatigue, edema, or irregular heartbeat occurs.
